Lab or test results as reported
- Lab results from last week show elevated blood glucose levels and high cholesterol.

- Patient reports they are not very active and spend most of their day sitting.
**Weight history**
- Patient is 5'8" and weighs 210 lbs, with a BMI of 31.9. They have gained 10 lbs since their last visit three months ago. Ideal body weight is estimated to be 140-160 lbs.
Patient is concerned about their recent weight gain and elevated blood sugar levels. They express a desire to improve their diet and overall health.
**:::::::: NUTRITION EVALUATION ::::::::**
Inadequate intake of fiber and increased intake of processed foods and sugary drinks related to poor dietary choices evident in client report and recent weight gain.
- Patient reports eating a diet that is low in fiber, with frequent consumption of processed foods, sugary drinks, and fast food.
- Patient reports a vitamin D deficiency.
- Patient demonstrates a basic understanding of healthy eating principles but struggles with practical application.

**INTERVENTIONS:**
**Nutrition prescription:** Patient will follow a 1800-calorie diet with an emphasis on whole grains, lean proteins, fruits, and vegetables. Patient will limit processed foods, sugary drinks, and saturated fats.
Food and/or nutrient delivery: Patient will be encouraged to use a food diary to track their intake and will be provided with a sample meal plan.
**Nutrition Education:** Topics discussed included portion control, reading food labels, meal planning, and the importance of fiber and protein.
**Nutrition Counseling:** Motivational interviewing techniques were used to explore the patient's goals and barriers to change. Goal setting was used to establish realistic and achievable targets.
