I am writing to refer my patient, Sarah Jenkins, who has a known medical condition of essential tremor, with no known allergies. She has previously undergone an appendectomy in 1995. Her current medications include Propranolol 40mg twice daily, Vitamin D 1000 IU daily, and a daily multivitamin.
They presented to me today with the following problem which includes a significant worsening of her essential tremor over the past six months, now significantly impacting her activities of daily living such as writing, eating, and dressing. She reports the tremor is predominantly in her right hand and is present at rest and with action. She has tried increasing her Propranolol dose under her GP's guidance without significant improvement. We discussed the possibility of surgical intervention, including Deep Brain Stimulation (DBS) or Focused Ultrasound Thalamotomy, and the potential risks and benefits of each. She is highly motivated to explore these options further to improve her quality of life.
