Erfasst werden Personalien, Kostenträger, AHV-, Unfall- oder IV-Verfügungsnummer, die gewählte Therapieart – Physiotherapie, Medizinische Trainingstherapie, Hippotherapie, Gehbad/Schwimmbad, Wohnungs- oder Arbeitsplatzabklärung, Robotik – sowie Zusatzleistungen wie Domizilbehandlung, Behandlung an Sonn- und Feiertagen oder Schienenversorgung. Dazu kommen behandlungsrelevante Diagnosen, Behandlungsziel, Limiten und Kontraindikationen, GLN, Datum und Unterschrift. Seite 2 dient als Kostengutsprachegesuch für Langzeitbehandlungen ab 37 Sitzungen mit ärztlicher und physiotherapeutischer Einschätzung.
