Session Plan: Clinical Interview & Assessment
MSE: The client arrived on time. The client appeared well-groomed and maintained good eye contact throughout the session. The client's mood was reported as anxious, with a congruent affect. The thought process was logical and coherent, with no delusions or hallucinations noted. The client was oriented to time, place, and person, with intact memory and concentration.
Risk: No risk of harm to self or others was observed at the time of this assessment.
Key issues addressed: Confidentiality (with limits) discussed in addition to practice payment, cancellation, and Court reporting.
Presenting Problems:
- The client reported experiencing persistent anxiety and difficulty sleeping, which have been ongoing for several months. The client is seeking therapy to manage these symptoms and improve overall well-being.
Predisposing Factors:
- The client has a family history of anxiety disorders and reported a childhood characterized by high levels of stress and parental conflict.
Precipitating Factors:
- The client recently experienced a job loss, which has exacerbated feelings of anxiety and uncertainty about the future.
Perpetuating Factors:
- The client engages in negative self-talk and avoids social situations, which reinforces feelings of isolation and anxiety.
Protective Factors:
- The client has a supportive partner and engages in regular physical exercise, which helps manage stress levels.
Formulation:
- The client's anxiety appears to be influenced by a combination of genetic predisposition, recent life stressors, and maladaptive coping strategies. Patterns of avoidance and negative thinking are prominent in the client's experiences.
Treatment Plan:
