Patient presents to the office today to discuss:
1. Frequent urination
2. Blood in urine
3. Lower abdominal pain
**"HPI TODAY:" 11/01/2024:**
The patient is a 58-year-old male who reports experiencing frequent urination, hematuria, and lower abdominal pain for the past two weeks. He states, "I feel like I need to go to the bathroom all the time, and it's really uncomfortable." A CT scan of the abdomen and pelvis was performed on 10/25/2024 at City Hospital. I personally reviewed and independently interpreted the scan and agree with the findings, which showed mild bladder wall thickening. The patient works as an accountant and enjoys gardening in his free time.
PMH, PSH, MEDS, ALLERGIES, SH, and FH:
- Information reviewed with patient and in EMR, with changes made where appropriate.
- PMH: Hypertension, Type 2 Diabetes
- PSH: Appendectomy
- MEDS: Lisinopril, Metformin
- ALLERGIES: Penicillin
- SH: Non-smoker, occasional alcohol use
- FH: Father with history of prostate cancer
**"ASSESSMENT AND PLAN:"**
