Recognised Disability, Other Conditions & Current Health:
The patient has a history of cerebral palsy and underwent corrective surgery for scoliosis at age 12. Currently, the patient experiences muscle stiffness and difficulty with fine motor skills. There is no history of mental health issues or substance use. The patient has not been hospitalized recently.
Medication Management:
The patient is on Baclofen for muscle spasticity, which is administered orally. Medications are stored in a secure cabinet and prescriptions are managed by the primary care physician.
Social History and Supports:
The patient lives with their parents and receives daily support from them. They also have a support worker who visits twice a week. The patient has no forensic history.
Education and Employment:
The patient completed high school and is currently enrolled in a vocational training program.
Cognition:
The patient demonstrates good cognitive function with no issues in memory or recognition skills.
Communication:
The patient communicates effectively using both verbal and non-verbal cues.
Mobility:
Indoors, the patient uses a walker and requires minimal assistance. Outdoors, they use a wheelchair and need assistance for transfers. The patient has a history of falls due to balance issues.
Transfers:
The patient requires assistance for bed and chair transfers using a transfer board. They can independently transfer in the shower with a grab bar and need assistance for toilet transfers.
Personal Activities of Daily Living:
The patient requires assistance with showering and dressing, using adaptive clothing. They can feed themselves with modified utensils and have a consistent sleep pattern.
Instrumental Activities of Daily Living:
The patient participates in meal planning with assistance and can prepare simple meals. They require help with grocery shopping and laundry tasks.
Home Environment:
The patient lives in a single-story home owned by their parents, with three bedrooms and two bathrooms. The living area is accessible with a ramp.
Goals:
The patient's goals include improving independence in daily living activities and enhancing mobility. Occupational therapy will focus on fine motor skills and adaptive techniques, with sessions twice a week for six months.
