Ideal Body Weight Calculator Key Points

The Ideal Body Weight Calculator applies the Devine formula, first published in 1974 as a practical tool for gentamicin dosing. For women, IBW = 45.5 kg + 2.3 kg for each inch of height over 60 inches; for men, the base is 50 kg with the same 2.3 kg increment per inch.The formula was not built from measured body composition. It is a clinical convenience formula developed for drug dosing purposes. While the broader concept of "ideal weight" was informed by Metropolitan Life Insurance actuarial tables from the mid-20th century (which linked height-weight ranges to longevity data in insurance applicants) the exact derivation of Devine's specific coefficients from those tables has not been clearly established in the primary literature. It has never been validated in children, amputees, or patients at extremes of height, so clinical judgment governs any weight-based dosing decision drawn from it.
