APC Medical Report for Insurance

Dear Aviva Insurance, I, Dr. Eleanor Vance, have been the treating General Practitioner for John Smith, born 12/03/1978 at The Grove Medical Practice, London since 10/06/2023. Please see my responses below pertaining to all questions asked of me as the provider, answered to the best of my knowledge on reviewing all clinical notes and results that I have access to at this clinic. He presented with headaches and reported a family history of hypertension. Examination revealed an elevated blood pressure of 160/95 mmHg. Investigations included a full blood count, renal function tests, and an ECG, all of which were within normal limits except for the elevated blood pressure. He was referred to a cardiologist for further evaluation. Treatment provided included lifestyle advice (diet and exercise) and medication (Lisinopril 10mg daily).
