Dear Dr Vance,
I am writing to provide a progress review for Jane Doe following her engagement in psychological therapy under the Medicare Better Access Initiative. Jane’s most recent score on the DASS-21 indicated that she was in the moderate range for depression, mild range for anxiety, and moderate range for stress.
Jane presented with symptoms of low mood, difficulty sleeping, and social withdrawal. She reported feeling overwhelmed by work and relationship stressors. Initial assessment revealed a history of childhood trauma and a tendency towards negative self-talk.
* Jane has made significant progress in identifying and challenging negative thought patterns.
* She has implemented relaxation techniques to manage anxiety symptoms.
* Jane has improved her communication skills and is actively working on setting boundaries in her relationships.
Jane continues to experience occasional low mood and is working on developing coping strategies for managing stressful situations.
Interventions utilised during therapy sessions included Cognitive Behavioural Therapy (CBT) techniques, mindfulness exercises, and psychoeducation regarding trauma and its impact.
