Second Opinion Cinical Request

Dear Dr. Eleanor Vance, I am writing to request a second opinion regarding Emily Carter, who has been diagnosed with Major Depressive Disorder. Despite a therapeutic trial of Sertraline 100mg daily for six weeks and an increase in Fluoxetine 40mg daily two weeks ago, Emily has shown minimal therapeutic benefit and continues to experience persistent low mood, anhedonia, and social withdrawal. Emily's current treatment regimen has not yielded the desired improvement, and she remains withdrawn and tearful. She maintains some level of motivation for therapy but reports no significant improvement in her overall mood or functioning. Given the lack of response to pharmacotherapy, I am considering Transcranial Magnetic Stimulation (TMS) as a potential treatment option for Emily. We have discussed TMS as a non-invasive treatment with no risk of memory loss, unlike Electroconvulsive Therapy (ECT), and Emily is willing to pursue this option. I would greatly appreciate your expert opinion on the suitability of TMS for Emily and any recommendations you may have for her treatment plan. Emily is currently on long-term disability leave and is in the midst of divorce proceedings, which have added to her stress and overall mental health burden. I include past correspondence to her General Practitioner and Treatment Team for clinical background in your assessment.
