Dear GP Dr. Smith,
Diagnosis: Basal cell carcinoma.
Treatment History: The patient has previously undergone cryotherapy for actinic keratosis.
Past Medical History: The patient has a history of hypertension and is fitted with a pacemaker.
Drug History: The patient is currently taking lisinopril and aspirin.
Allergy History: The patient is allergic to penicillin.
Social History: The patient has significant sun exposure due to outdoor occupation and has a Fitzpatrick skin type II.
Clinical Findings:
John Doe, a 58-year-old male, was reviewed in the dermatology clinic today regarding a lesion that has been present for six months with recent rapid growth and occasional bleeding on the left cheek.
On examination, the lesion is a pearly, nodular lesion with telangiectasia, measuring approximately 1.5 cm in diameter on the left cheek. There are signs of malignancy.
Management Plan:
• The lesion is clinically consistent with basal cell carcinoma.
• I have arranged for excision, and we will update you accordingly.
Follow-Up:
The patient will be reviewed in 3 months post-excision.
If there are any new concerns, please feel free to refer the patient back.
