Reason for Visit: You visited our clinic due to persistent eczema on your arms and legs, which has been worsening over the past few months despite over-the-counter treatments. Medical History: You have a history of asthma and seasonal allergies, with no previous surgeries. Medications: You are currently taking loratadine for allergies and using a hydrocortisone cream for eczema. Allergies: You have a known allergy to penicillin. Social History: You are a non-smoker and consume alcohol occasionally. You work as a software engineer and have a sedentary lifestyle. Examination Findings: On examination, there were erythematous, scaly patches on the extensor surfaces of the arms and legs, consistent with eczema. Investigations: No additional investigations were ordered at this time. Diagnosis: The clinical findings are consistent with atopic dermatitis. Management Plan: I have prescribed a stronger topical corticosteroid to be applied twice daily. Additionally, I recommend using a fragrance-free moisturizer and avoiding known irritants.
