For context, while RBT session notes are specifically written by RBTs, they contribute to the broader ABA therapy documentation insurers require to approve reimbursements. Some of the most basic information insurers look for includes the name of the service provider(s), when and where the service was provided, and the specific interventions practiced during the billed session, all of which are found in the RBT session notes.

Avoid writing about emotions or assumptions, as well as loaded or subjective terms like “uncooperative” or “acting out.” For example, instead of writing “client was frustrated”, note something like “client cried for 3 minutes after the task was introduced.”

Tip #2 - Align Notes with Treatment Goals and Programs

Document the specific program followed during the session, and provide factual observations regarding the client’s progress or setbacks based on the BCBA-approved goals. Instead of writing “Client did well today,” write something like “Client independently responded to 8/10 trials with verbal prompt,” providing clear and unbiased insight to third-party reviewers.

Tip #3 - Keep it Concise but Complete

RBT session notes are expected to be thorough, but not wordy. Avoid filler language or “fluff” that doesn’t provide any useful insights into the documented sessions, and write only clear and objective facts to avoid misinterpretation and potential claim delays or denials.

“Client was in a good mood today. We worked on some tasks, and they seemed to enjoy the session. There were a couple of behavioral issues, but nothing major.”

Good Example:
“Ran a manding program, and the client requested to do them independently on 6/10 trials. One elopement, redirected with verbal prompt.”
