Toilet Fitting
The toilet ordered for the patient did not fit due to the unique shape and size of the existing toilet. The current toilet is oval-shaped with a height of 16 inches, which is not compatible with the standard fittings.
Bench Seat
The bench seat ordered does not meet the patient's needs as it does not accommodate the patient's current transfer process, which involves using a walker and a transfer board. It is suggested to use a sliding transfer bench in addition to the current bench seat to facilitate safer transfers.
Floor to Ceiling Pole
A floor to ceiling pole has been ordered and will be installed to assist the patient with transfers. The patient will use the pole in conjunction with the bench for stability during transfers. There are concerns about the wheelchair positioning and space constraints in the bathroom, but the decision has been made to order the pole and trial it before the patient uses it independently.
Patient's Current Standing Ability
The patient currently has limited standing ability and requires strength building. There are concerns about safety during transfers and the risk of falls in the bathroom. A plan is in place to assess the patient's standing and stepping ability before using the new equipment.
Personal Support Worker (PSW) Scheduling
There have been issues with inconsistent PSW arrival times, impacting the patient's routine. Reliable PSWs have been identified, and a request has been made for them to be scheduled consistently. The patient's morning routine is heavily reliant on timely PSW assistance, and communication with the PSW supervisor will be initiated to request a consistent arrival time.
Equipment Funding
Potential funding sources for the necessary equipment have been discussed. The paperwork and application process for funding have been outlined, and additional equipment needs have been identified.
Toilet and Bench Seat Alternatives
A plan is in place to research alternative toilet options and consult with the equipment provider. Alternative bench seat options are also being considered, with the possibility of using funding for these alternatives.
Physiotherapy
A physiotherapy assessment and treatment plan have been discussed. There are concerns about the limited number of approved visits and the need for ongoing therapy to build the patient's strength and mobility.
Next Steps
The plan includes ordering and installing the necessary equipment, following up on alternative equipment options, coordinating with the PSW supervisor for consistent scheduling, assisting with the funding application for additional equipment needs, and monitoring the patient's progress with physiotherapy to adjust the plan as needed.
