Subjective: - The patient, a 7-year-old male, was brought in by his mother due to concerns about stuttering and difficulty in forming complete sentences. The mother reported that these issues have been present since the child started school. - The patient has a history of frequent ear infections but no previous surgeries. - Currently, the patient is not on any medications or herbal supplements. - The family recently moved to a new city, which has been a significant change for the child. - No known allergies. Observations: - During the session, the patient demonstrated moderate stuttering, particularly at the beginning of sentences. His vocabulary is age-appropriate, but he struggles with sentence structure. - The patient appeared anxious and fidgety, especially when asked to speak in front of others. - No physical abnormalities were noted that would affect speech. - Cognitive-linguistic skills are within normal limits for his age. - The patient was cooperative and engaged well with the activities presented. Recommendations: - Therapy goals include reducing stuttering frequency and improving sentence structure. - Recommended techniques include fluency shaping and gentle onset exercises. - Home exercises include daily reading aloud with a focus on slow, deliberate speech. - A follow-up appointment is scheduled for two weeks from today. - Additional recommendation for the parents to encourage the child to participate in group activities to build confidence.
