Diagnosis: Right knee meniscal tear, secondary to a sporting injury. Plan: Recommend arthroscopic meniscectomy. Arrange pre-operative physiotherapy. History: John presented today with a chief complaint of right knee pain following a rugby match. He reports a twisting injury to the knee. John is currently employed as a teacher. His leisure interests include rugby and hiking. His wife attended the appointment with him. Associated symptoms include locking and giving way of the right knee. Functional limitations: John is experiencing difficulty with walking, running, and squatting. Treatment to date: John has been taking over-the-counter pain relief and has been icing his knee. He has not received any formal physiotherapy. Past medical history: John has no significant past medical history. He has no known allergies. His family history is unremarkable. He is a non-smoker and drinks alcohol socially. Medications: Ibuprofen 400mg as required. Examination: Observations: John is able to weight bear, but is favouring his right leg. Examination of the right knee revealed a positive McMurray's test and joint line tenderness. Imaging: MRI of the right knee confirmed a medial meniscal tear. Management: Arthroscopic meniscectomy is planned. Pre-operative physiotherapy will be arranged. John will be provided with information regarding post-operative care and rehabilitation.
