Letter to Referrer

Dear Dr. Emily Johnson, I am writing to provide an update on, John Doe, who was referred to me for evaluation of persistent headaches and dizziness. John presented with ongoing severe headaches, dizziness, and occasional blurred vision. He reported that these symptoms have been progressively worsening over the past three months. During our discussion, he mentioned a history of migraines and a recent increase in stress levels due to work. Upon evaluation, the following findings were noted: neurological examination revealed mild nystagmus and decreased coordination on the left side. MRI of the brain showed a small lesion in the right cerebellum. Blood tests were within normal limits. The patient’s past medical history includes: hypertension and a tonsillectomy performed in childhood. Current medications and supplements: Lisinopril 10mg daily, Omega-3 supplements. Allergies: Penicillin. Social history: John is a 45-year-old software engineer, married with two children, and a non-smoker. He consumes alcohol occasionally. Based on the assessment, the following plan has been initiated: referral to a neurologist for further evaluation of the cerebellar lesion, continuation of current antihypertensive therapy, and initiation of a headache diary to monitor symptoms. A follow-up MRI is scheduled in three months. I will continue to monitor John closely and provide updates as necessary.
