Initial Oncology consult

Dear Dr. Smith, Thank you for asking me to meet with this patient for further management of metastatic breast cancer. As you know, Jane Doe is a 52-year-old female. She presented with persistent fatigue and a palpable mass in the left breast. Investigations done so far include a mammogram, which revealed a suspicious lesion, and a biopsy confirming invasive ductal carcinoma. Additionally, a CT scan showed metastasis to the liver. PAST MEDICAL HISTORY Jane has a history of hypertension and type 2 diabetes, both of which are well-controlled with medication. MEDICATIONS Jane is currently taking Lisinopril for hypertension and Metformin for diabetes. She has also been started on Letrozole as part of her cancer treatment. Drug allergies: Jane has no known drug allergies. SOCIAL HISTORY Jane is a non-smoker and consumes alcohol occasionally. She works as a school teacher, is married, and has two children. She lives with her family in a suburban area. PHYSICAL EXAMINATION On examination, Jane appears well-nourished and in no acute distress. There is a palpable mass in the left breast, approximately 3 cm in diameter, with no axillary lymphadenopathy. Her vital signs are stable. IMPRESSION Jane has metastatic breast cancer with liver involvement. She is currently undergoing hormonal therapy and will be evaluated for further systemic treatment options.
