It was a pleasure to review Mr. David Miller, a 58 year old Accountant who presents with abdominal pain and bloating. My assessment is: - Irritable Bowel Syndrome (IBS) - based on patient history of recurrent abdominal pain, bloating, and altered bowel habits, with no alarm features. - Rule out coeliac disease - patient has reported some symptoms that could be related to coeliac disease, and a blood test has been requested. Our plan going forward is: - Advised Mr. Miller to follow a low FODMAP diet for 6 weeks and to monitor his symptoms. - Ordered blood tests to rule out coeliac disease. - Scheduled a follow-up appointment in 8 weeks to review the results of the blood tests and assess the effectiveness of the dietary changes. "I explained the risks and benefits of the blood tests, including the possibility of false positives or negatives, and the need for further investigations if the results are abnormal. Mr. Miller had the opportunity to ask questions regarding the procedure." Salient points for noting are: - Patient reports abdominal pain, bloating, and altered bowel habits for the past 6 months. - Symptoms are worse after meals and are not associated with weight loss or bleeding. - Patient denies any family history of bowel cancer. Past medical history: - No significant past medical history. - No previous surgeries. Medications and herbal supplements: - Omeprazole 20mg daily for heartburn. - No herbal supplements. Social history: - Non-smoker. - Drinks alcohol occasionally. - Works as an accountant. Allergies: - No known allergies. Examination findings were: - Abdomen soft, non-tender to palpation. - No masses or organomegaly. - Bowel sounds present. Important investigation results were: - Blood tests pending.
