GP 1st letter

I hope this letter finds you well. Thank you for referring Mrs. Sarah Jones and Mr. David Jones to me for a consultation regarding their fertility journey. Mrs. Jones and Mr. Jones came to see me for assessment and management of primary infertility. They have been trying to conceive for 18 months. Mrs. Jones has a history of irregular periods and was diagnosed with polycystic ovary syndrome (PCOS) five years ago. Mr. Jones has no significant medical history. They have not undertaken any treatment previously. I have proposed the following investigations: a full blood count, thyroid function tests, and a pelvic ultrasound for Mrs. Jones. I have also requested a semen analysis for Mr. Jones. We discussed treatment options including ovulation induction with clomiphene citrate, intrauterine insemination (IUI), and in vitro fertilisation (IVF). I explained the advantages, disadvantages, and success rates of each option. We discussed that the success rate for IUI is approximately 15-20% per cycle, while IVF has a higher success rate, around 40-50% per cycle, depending on the woman's age and other factors. We also discussed the potential risks associated with each treatment. I recommended that they undergo the investigations I have proposed and then return for a follow-up appointment to discuss the findings and make an informed decision on the optional treatment. I plan to conduct a follow-up appointment with Mrs. Jones and Mr. Jones in 4 weeks to discuss the findings of their investigation and screening tests so they could make an informed decision on the optional treatment.
