**Current Issues** 1. Recurrent episodes of acute sinusitis. 2. Persistent nasal congestion and postnasal drip. 3. Reduced sense of smell. **Background** * Allergic rhinitis. * Previous tonsillectomy (2010). * Previous sinus CT scan (2023): showed mild mucosal thickening in the maxillary sinuses. * Past Medical History: Allergic Rhinitis. **Audiometry** Audiometry on 10 October 2024 showed mild bilateral sensorineural hearing loss. Compared to the previous audiogram from 2023, there has been a slight worsening in the high frequencies. **Impression** - Chronic sinusitis with possible allergic component. **Plan** - Continue nasal steroid spray. - Consider referral to allergy clinic for further evaluation. - Follow-up in 3 months. **Requested GP Actions** - Repeat prescription for nasal steroid spray. **Assessment** “Mr” Smith returned to our ENT clinic today following a previous visit on 10 September 2024 for evaluation of recurrent sinusitis and nasal congestion. Current status: Mr. Smith reports persistent nasal congestion, postnasal drip, and a reduced sense of smell. He has been using nasal steroid spray as prescribed, with some improvement in symptoms. He denies fever, facial pain, or headache. He reports no new allergies. He has a history of allergic rhinitis. Examination Findings: Nasal examination revealed mild mucosal swelling and clear rhinorrhea. Tympanic membranes were normal bilaterally. We will continue with the current management plan and schedule a follow-up appointment in three months.
