Dear Complaints Team, I can confirm I have investigated the concerns raised regarding John Smith, Hospital number 1234567, DoB 01/01/1980. Mr Smith attended A&E on 01 November 2024 via 999 ambulance. He arrived at 14:00. Mr Smith was seen by the nurse soon after arrival. The doctor saw and assessed Mr Smith at 14:30. The reason for attending A&E was chest pain. His current medication and past medical history were duly noted. * Medication: Aspirin 75mg daily, Ramipril 2.5mg daily. * Past Medical History: Hypertension, previous MI. The doctor reviewed Mr Smith's medical background from the electronic records. The predominant symptom at that time was chest pain. Following the assessment and review of the patient's presentation, the concerns raised related to the time taken to administer pain relief and the communication regarding the need for further investigations. The learning points from this case include the importance of timely pain management and clear communication with patients regarding diagnostic processes.
