The patient had a coronary artery bypass graft surgery five years ago. He is currently taking atorvastatin and lisinopril. The patient denies any herbal supplement use. He has a 20-pack-year smoking history but quit 10 years ago. No known drug allergies. Objective: - Vital signs: Blood pressure 140/90 mmHg, heart rate 88 bpm, respiratory rate 18 breaths per minute, temperature 98.6°F. - Physical examination reveals a regular heart rhythm with no murmurs, rubs, or gallops. Mild peripheral edema noted in the lower extremities. - Recent echocardiogram shows left ventricular hypertrophy and an ejection fraction of 45%. Assessment: - Diagnosis: Ischemic heart disease with reduced ejection fraction. - The clinical findings suggest worsening heart function, likely due to progression of coronary artery disease. Plan: - Initiate treatment with beta-blocker therapy and increase the dose of lisinopril. - Recommend lifestyle modifications including a low-sodium diet and regular cardiovascular exercise. - Schedule a follow-up appointment in four weeks to reassess symptoms and medication efficacy. - Referral to a cardiologist for further evaluation and management. - Provided patient education on recognizing symptoms of heart failure and the importance of medication adherence.
