Begin letter with: It was nice to see Mr. John Smith again for his follow up appointment.
Dear John,
I am writing to you following your recent appointment on 1 November 2024.
Your recent echocardiogram showed that your heart function is good, with an ejection fraction of 62% (this means your heart is pumping blood efficiently). There is also mild mitral regurgitation (a slight leak in one of your heart valves), which is not currently causing any problems.
You are currently taking Atorvastatin, Bisoprolol and Ramipril. We will continue with these medications.
Your cardiovascular risk factors include a history of smoking (quit 5 years ago), and well-controlled hypertension. Your cholesterol profile is within the target range. There is a family history of heart disease on your father's side.
Your past medical history includes hypertension and hypercholesterolemia.
Examination findings: Heart rate 72 bpm, BP 130/80 mmHg, O2 sats 98% on room air. Cardiac auscultation: normal S1 and S2, no murmurs. No signs of fluid retention.
In conclusion, your heart function is good, and your current medications are appropriate. We will continue with your current medications. I recommend you continue with your healthy lifestyle and follow up in 6 months. Please contact the clinic if you experience any new symptoms.
Yours sincerely,
Dr. Thomas Kelly
