Immunotherapy:
- Patient has been on subcutaneous immunotherapy (SCIT) for 2 years with moderate improvement in symptoms. Wishes to continue treatment.
HPI:
- Patient reports increased nasal congestion, sneezing, and itchy eyes over the past 3 weeks. Symptoms are severe in the morning and improve slightly with antihistamines.
Current Medications:
- Loratadine 10 mg once daily
- Fluticasone nasal spray 2 sprays in each nostril daily
Physical Exam:
- General appearance: Alert and oriented, no acute distress
- Nasal findings: Turbinate hypertrophy, clear rhinorrhea
- Throat: Mild erythema, no exudate
- Eyes: Conjunctival injection, watery discharge
- Respiratory: Clear to auscultation bilaterally

- Skin: No rashes or urticaria
Test Results:
- Recent skin prick test shows positive reactions to grass and tree pollen

- Continue SCIT
