BODY OF LETTER: I was pleased to review John Smith, a 68-year-old male, today with regard to increasing difficulty with urination and frequent nocturnal micturition. John Smith has a past medical history significant for controlled hypertension and type 2 diabetes. He presented with a 6-month history of worsening lower urinary tract symptoms, including hesitancy, weak stream, nocturia (waking 3-4 times per night), and a sensation of incomplete bladder emptying. He denies any haematuria, dysuria, or fever. His IPSS score was 22. On examination, his abdomen was soft and non-tender, with no palpable masses. Digital rectal examination revealed a moderately enlarged, smooth, and firm prostate gland, estimated to be 40-50 grams, without any suspicious nodules. There were no signs of acute infection or palpable bladder distension. My impression is that this is benign prostatic hyperplasia causing bothersome lower urinary tract symptoms. We have decided to commence alpha-blocker therapy with Tamsulosin to improve urinary flow and reduce storage symptoms. I have advised Mr. Smith to monitor his symptoms, avoid excessive fluid intake before bed, and seek medical attention if he develops fever, severe pain, or inability to pass urine. We will review his progress and results of planned investigations in 6 weeks. I answered all of their questions and concerns and the patient was happy with the plan above.
