Das Dokument ist das Schweizer Verordnungsformular für Ergotherapie. Ein Arzt oder eine Ärztin verordnet darauf eine ergotherapeutische Behandlung und hält fest, über welchen Kostenträger sie läuft: KVG, UVG/MVG oder IVG, inklusive Art und Umfang der Behandlung (Abklärung, einzelne oder mehrere Behandlungsserien, Langzeitbehandlung). Neben den Personalien des Patienten und der Adresse der Ergotherapiepraxis erfasst das Formular den Behandlungsort (Zentrum/Praxis, zu Hause, Institution), das Behandlungsziel (körperliche Funktionen, psychische Erkrankung, Sturzprävention), die Ursache (Krankheit, Unfall, Geburtsgebrechen) und die Diagnose. Zum Schluss bestätigen der verordnende Arzt oder die Ärztin und der behandelnde Ergotherapeut oder die Ergotherapeutin die Verordnung je mit ZSR-Nummer, GLN-Nummer, Datum und Unterschrift.
