Orthopedic physicians and care teams are among the most administratively burdened in medicine. Between overseeing millions of joint replacements, spine surgeries, and trauma repairs, plus pre-op visits, follow-ups, and Ambulatory Surgery Center (ASC) billing, there is little room left in the day for paperwork.

However, no two ambient AI scribes are the same. For organizations weighing whether to build or buy, it pays to vet the options carefully. That starts with asking the right questions.

Here is what orthopedic practices should ask to start the ambient AI vetting process:

#1: What is the quality and design of the clinical AI scribe? Orthopedics covers enormous ground, from a five-minute post-op check to a complex spine consult, so look for clinician-driven design that adapts to your templates and the pace of your clinic. It should be intuitive enough that a surgeon, PA, or physical therapist can pick it up and start using it the same day.

#2: What does it cost, including startup costs? Some ambient AI tools come with steep licensing fees, long rollout timelines, and constant IT involvement, costs that hit especially hard for independent practices and ASCs already managing tight margins. Look for transparent pricing and a low barrier to entry, so your team is documenting visits within days, not months.

#3: How does the ambient AI solution support compliance? Orthopedic documentation carries its own weight, from surgical consent to ASC-specific billing requirements, on top of HIPAA and state regulations. The right AI care partner should make it easier to stay compliant, not add another layer of things to track.

#4: How does the ambient AI solution approach security? Know exactly how and where your data is stored and how the tool handles protected health information (PHI). For a specialty that moves between clinic, OR, and ASC, this is the best way to cover your bases.

#5: How well does it work with our other technology systems? An ambient AI scribe needs to work with the systems your practice already runs on. Does it integrate with most EHRs? If your tech stack is a little unusual, will it work just as well? Not connecting to what you already have will create more work, not less.

Most practices will have plenty of follow-up questions, but these five are a solid starting point for narrowing a crowded field down to the one or two scribes worth a real demo.
