MAiD Assessment progress note

MAiD Assessment progress note I met with Sarah again. This was in order to continue my assessment of her eligibility for medical assistance in dying (MAiD). During this follow-up appointment, we discussed her understanding of her medical condition, advanced prostate cancer with widespread metastases, and the prognosis. She reiterated her consistent and unwavering request for MAiD, stating that her suffering is intolerable and cannot be alleviated by means acceptable to her. We reviewed her palliative care options again, including current pain management strategies and psychological support, and she confirmed that these were insufficient to meet her needs. She presented a clear and informed decision, demonstrating a full appreciation of the nature and consequences of MAiD. We also touched upon her family's understanding and support for her decision, noting that they are aware and respectful of her wishes. We concluded that she continues to meet the eligibility criteria for MAiD. We reviewed the next steps in the process, including the requirement for a second independent assessment, which she understands and is prepared for. A follow-up appointment has been scheduled for 1 November 2024 to facilitate this second assessment.
