Dear Dr. Eleanor Vance,
Re: Mr. Arthur Jenkins
"Thank you for seeing the patient below."
I am writing to refer my patient, Mr. Arthur Jenkins, who is known with osteoarthritis of the left knee, hypertension, and well-controlled type 2 diabetes, and is currently using Amlodipine 5mg daily, Metformin 500mg twice daily, and over-the-counter paracetamol for pain relief.
He presented to me today with the following problem which includes increasing left knee pain over the last 6 months, significantly impacting his mobility and daily activities. The pain is worse with weight-bearing and climbing stairs, and he reports morning stiffness lasting approximately 30 minutes. Physical examination revealed crepitus and limited range of motion in the left knee, with mild swelling. Conservative management including physiotherapy and NSAIDs has provided only temporary relief, and he is now struggling to walk more than 100 meters without significant discomfort. We discussed the progressive nature of his condition and the potential for surgical intervention, and he is keen to explore all available options.
Your expertise is be greatly appreciated. I look forward to your opinion.
"Kind regards,"
Dr. Jonathan Reid
