FRACTURE CLINIC Working Diagnosis: Left distal radius fracture, 28 October 2024, fall onto outstretched hand (FOOSH) mechanism. PLAN: 1. Continue non-weight bearing on left wrist. 2. Maintain current cast for another 3 weeks. 3. Commence gentle finger and elbow range of motion exercises. 4. Arrange follow-up appointment for cast removal and review with orthopaedics. 5. Prescribe paracetamol 1g QDS PRN for pain. SUMMARY: Today: Patient presented today for their initial follow-up after sustaining a left distal radius fracture. The patient reports pain is well-controlled with paracetamol and denies any neurovascular compromise or increased swelling. Cast remains intact and comfortable. Examination: Left upper limb: Cast intact, no signs of skin irritation or pressure areas. Capillary refill time <2 seconds in all digits. Sensation intact to light touch in radial, median, and ulnar nerve distributions. No tenderness over uncasted areas. Full, pain-free range of motion in shoulder and elbow. Fingers show good active and passive range of motion. Discussion: Discussed the current healing progress and the importance of cast care. Reviewed exercises for fingers and elbow to prevent stiffness. Explained the timeline for cast removal and the next steps for rehabilitation.
