Chiropractor's 1:1 Video Consultation Note Subjective: Patient reports feeling much better this week. Currently at week 3, day 2 in the app. Exercises are helping with back pain. Applied all advice given regarding posture correction. Mentioned attending their grandchild's school play next week. Reports 50% reduction in lower back pain. No new symptoms or concerns. Discussion and Education: Discussed the importance of maintaining proper ergonomics at work. Educated on the benefits of regular stretching. Provided guidance on safe lifting techniques. Analysis: Pt Progress: Patient shows good progress with symptom reduction. DHI score improved from 25% to 10% since last visit, indicating significant functional improvement. To Note: Patient prefers morning appointments. Responds well to visual demonstrations of exercises. Working Diagnosis and Hx Summary: Working Diagnosis: Lumbar disc herniation (L4-L5). Patient presented initially with acute lower back pain radiating down the right leg, exacerbated by prolonged sitting. Initial consultation revealed positive straight leg raise test. Chiropractic adjustments and prescribed exercises have been effective in managing symptoms. Plan: Continue current exercise programme for another week. Progress to week 4 in the app. Focus on core strengthening exercises. Advised to use a lumbar support cushion. Recommended performing stretches twice daily. Follow-up booked for 1 November 2024.
