Dear Dr. Eleanor Vance:
Ethan Miller is a 8-year-old male who was seen on 1 November 2024 in the outpatient pediatric learning and behavior clinic, accompanied by his mother, Mrs. Sarah Miller, for follow up of Attention-Deficit/Hyperactivity Disorder (ADHD). Ethan Miller is currently medically managed with Methylphenidate ER 18mg daily.
Ethan is currently in Year 3 at Willow Creek Primary School. Mrs. Miller reports that he is struggling with staying focused during lessons and often misses instructions. His last school report indicated difficulties with task completion and organisation. We are still awaiting the teacher Vanderbilt assessment scales.
Psychoeducational testing was completed by Dr. Anya Sharma, the school educational psychologist, in September 2024. Dr. Sharma's report highlighted significant challenges with sustained attention and executive functioning. We have communicated with Dr. Sharma regarding Ethan's current progress and future accommodation needs.
At home, Mrs. Miller observes that Ethan is highly energetic and frequently interrupts conversations. He has difficulty following multi-step commands and often leaves tasks unfinished, such as homework and tidying his room. His impulsivity sometimes leads to minor arguments with his younger sister.
During this visit, the parent Vanderbilt ADHD Diagnostic Rating Scale was reviewed. Ethan scored significantly in areas related to inattention and hyperactivity/impulsivity, consistent with his ADHD diagnosis. No other formal assessments were completed today.
Mrs. Miller expressed general satisfaction with Ethan's current medication, noting an improvement in his ability to focus on tasks for longer periods. She has not observed any significant side effects, though she mentioned he occasionally has a reduced appetite in the morning, which resolves by lunchtime.
PHYSICAL EXAMINATION
Ethan's physical examination revealed a weight of 28 kg (50th percentile for age), height of 130 cm (60th percentile for age), and a BMI of 16.5 kg/m² (55th percentile for age). Cardiorespiratory examination was unremarkable, with clear breath sounds bilaterally and regular heart rhythm. Blood pressure was 95/60 mmHg. No other acute findings were noted.
IMPRESSION AND PLAN
In summary, Ethan Miller is a 8-year-old male with Attention-Deficit/Hyperactivity Disorder (ADHD). He is currently medically managed with Methylphenidate ER 18mg daily and we do not feel he needs to be increased at this time.
We suggest continuing to work closely with the school guidance counselor and Dr. Sharma, the educational psychologist, to ensure appropriate classroom accommodations are in place. These may include preferential seating, reduced distractions, and chunking assignments into smaller, manageable parts. We will also recommend a referral to an occupational therapist for further assessment of his organisational skills.
Ethan is scheduled for a follow-up appointment in three months' time, on 1 February 2025, to re-evaluate his medication efficacy and behavioural strategies. We will request updated teacher Vanderbilt scales prior to this visit.
