Sarah Jones 38 years old 1 November 2024 This document summarises your recent consultation regarding your premature ovarian insufficiency (POI). It aims to provide a clear overview of our discussion and the agreed-upon plan for your ongoing care. Reason For Consultation: The specific reason for this POI consultation visit was to review current symptom management, discuss long-term health implications, and adjust hormone replacement therapy. HRT: Previously on combined oestrogen and progesterone oral tablets, specifically Oestrogel 2 pumps daily and Utrogestan 100mg nightly for 12 days each month, initiated 18 months ago. Patient reports good adherence but continued experience of hot flushes. Menstrual History: Irregular periods commenced at age 35, with complete cessation of menses by age 36. The last menstrual period was approximately 2 years ago. Currently experiencing amenorrhoea. Current Symptoms: * Persistent hot flushes, 5-7 times daily * Night sweats affecting sleep * Increased vaginal dryness * Mild mood swings * Decreased libido Past Medical History: * Diagnosis of Premature Ovarian Insufficiency (POI) confirmed at age 36 via elevated FSH levels. * No other significant medical conditions. Family History: * Mother experienced early menopause at age 45. * Maternal aunt diagnosed with an autoimmune thyroid condition. Current Medications: * Oestrogel: 2 pumps daily * Utrogestan: 100mg nightly for 12 days a month Supplements: * Vitamin D: 1000 IU daily * Calcium: 500mg daily Social And Lifestyle History: * Non-smoker. * Consumes alcohol socially, approximately 2-3 units per week. * Engages in moderate exercise (walking, yoga) 3 times per week. * Diet is generally balanced, but admits to occasional sugary snacks. * Reports moderate stress levels related to work. Fertility History: Expressed ongoing fertility concerns and a desire to explore all options. Has not attempted conception since POI diagnosis but is interested in future possibilities. No fertility treatments initiated to date. Discussion: During our discussion, we reviewed Sarah's journey since her POI diagnosis. We spoke about the emotional impact of receiving this diagnosis at a younger age and the biological reasons behind premature ovarian insufficiency. We acknowledged the challenges she has faced and validated her experiences. We discussed her persistent hot flushes and night sweats, exploring how these symptoms impact her quality of life and sleep. Treatment options for symptom management were reviewed, including adjusting her current HRT regimen. Patient education focused on understanding the different types of HRT and their potential benefits and risks. We extensively covered the critical importance of managing low oestrogen levels at a younger age to protect long-term health. The discussion highlighted the protective role of hormones in maintaining bone density, cardiovascular health, and cognitive function. Sarah was keen to understand how these aspects are routinely assessed and managed. Our conversation also covered the specific clinical considerations related to her low oestrogen levels, including strategies to mitigate long-term health risks. We addressed her concerns about bone health and the need for regular screenings. Education was provided on the benefits of consistent HRT use in preventing osteoporosis and supporting overall well-being. The Effects On Bone, Brain And Heart Health And How This Should Be Assessed And Managed: We discussed the increased risk of osteoporosis, cardiovascular disease, and potential cognitive changes due to early oestrogen deficiency. Management strategies include regular bone density scans (DEXA), lipid profile monitoring, and maintaining adequate HRT. The Changes In Or Absence Of Periods And When Any Bleeding Might Need To Be Assessed By Your GP: We reviewed that the absence of periods is expected with POI, and discussed that any unexpected or heavy bleeding should be promptly reported to her GP for further assessment to rule out other causes. The Effects On Fertility And Need For Contraception: Sarah's fertility is significantly reduced due to POI; however, spontaneous pregnancies can rarely occur. We discussed the need for continued contraception if pregnancy is to be avoided, despite the low likelihood of natural conception. Emotional Support And Relationships: We explored the emotional toll of POI, including feelings of loss and grief. The impact on intimate relationships was acknowledged, and resources for emotional support and counselling were discussed. Symptoms You May Experience: We reviewed common POI symptoms such as hot flushes, night sweats, vaginal dryness, mood changes, and potential for long-term health risks without adequate oestrogen replacement. Vaginal And Urinary Health: Discussion included the management of vaginal dryness with local oestrogen therapy if needed, and the importance of maintaining good urinary health to prevent recurrent infections. The Important Role Of Hormone Replacement Therapy For Your Health As Well As Symptom Management: We emphasised that HRT is crucial not only for managing current symptoms but also for protecting long-term bone, heart, and brain health due to the early loss of natural oestrogen. Sarah's primary concerns during the consultation were the persistence of hot flushes despite current HRT and the long-term implications of POI on her bone health and cardiovascular system. She also expressed a desire to understand all available options for managing these issues effectively. Plan: Adjust Oestrogel dose from 2 pumps to 3 pumps daily to better manage hot flushes and night sweats. Continue Utrogestan 100mg nightly for 12 days a month. Arrange a DEXA scan within the next 3 months to assess current bone density and establish a baseline. Schedule a follow-up consultation in 3 months to review symptom control, HRT adherence, and DEXA scan results. Referral to a reproductive endocrinologist for further discussion regarding fertility preservation options.
