Specialty: General Practitioner Discussed with GP supervisor, Dr. Eleanor Vance. Patient presented with persistent fatigue, unexplained weight loss, and night sweats for the past three months. My assessment was to investigate for underlying malignancy or chronic infection. Planned initial blood tests including FBC, ESR, CRP, LFTs, U&Es, and a CXR. Dr. Vance concurred with the diagnostic approach and suggested also considering a thyroid function test given the fatigue, and arranged for a follow-up appointment in one week to review results. Discussed with GP supervisor, Dr. Eleanor Vance. Patient, a 4-year-old child, presented with recurrent ear infections, the third episode in six months. My assessment indicated a need to consider referral to ENT. Planned to prescribe amoxicillin for this current infection and advise on symptom management. Dr. Vance agreed with the current management but recommended a discussion with the parents regarding tympanostomy tubes if infections continue, and advised offering a referral to ENT if another infection occurs within the next two months.
