Background

Airrosti is a healthcare organization, based in Texas, known for its hands-on, evidence-based approach to musculoskeletal care. For two decades, its chiropractors and physical medicine providers have focused on getting patients back to movement through a unique one-hour appointment model: 30 minutes of manual therapy followed by 30 minutes of active rehabilitation. Clinicians like Dr. Gary Cagan Randall in Dallas–Fort Worth and Dr. Jason DeRoche in San Antonio each see up to 18 people a day, caring for patients ranging from competitive athletes to everyday active adults. What makes Airrosti stand out is its commitment to detail: every exam is functional, every plan individualized, and every patient interaction deeply hands-on.

Since integrating Heidi across the organization, Airrosti’s providers have generated over 230,000 sessions and produced more than 272,000 clinically structured notes. With an average adoption rate of 87% across 275 users, Heidi has quickly become part of the fabric of Airrosti’s workflow.

Challenges

At Airrosti, every minute counts. The team’s model relies on precision and presence, but their old systems made that difficult. Before Heidi, documentation meant hours of clicking and typing inside Raintree, a traditional EMR. “We basically have tabs to go through… click click click… and then wait for it to come back up,” said Dr. DeRoche. Even with efficient workflows, notes took three to five minutes each, adding up to two or three hours of admin after clinic hours. “I hated charting,” said Dr. Randall. “Ask any provider… charting, it’s terrible.”

For high-volume clinicians, there was a trade off. As Dr. Randall put it, “If I'm in a big workflow, I would have to decide, am I capturing data on a computer or am I going to go be a doctor?”

The constant toggling between doctor and data clerk took its toll. “Most of us used to have to sit here and type while speaking with the patient,” recalled Dr. DeRoche. “It can appear that I'm disengaged.” With 16–19 patients a day, even the most experienced providers struggled to capture every nuance. “Do you think I'm gonna remember that Jane Doe had a small pinching at the proximal portion of rectus… during hip flexion? No… now [with Heidi] it's in there,” said Dr. Randall.

For years, clinicians tried to reclaim their time by blocking schedules, working through lunch, or finishing notes long after the last patient left. But the strain was constant, and the balance between accuracy and humanity felt impossible.

Solution

That changed when Airrosti partnered with Heidi. Dr. DeRoche joined the early pilot, curious but cautious. “I like innovation. I like new products. I like to test new things,” he said. “It’s pretty intuitive. It took all of two patients to figure it out.” Dr. Randall first saw Heidi in action during a live demo. “I saw him demo it… and then they hit stop transcribing and it just [filled the template] and I was like okay sign me up.” From that day forward, he said, “it’s seamless for me.”

Now, both clinicians start every visit with Heidi running in the background. Patients are informed upfront. From there, the conversation flows naturally. Dr. DeRoche said, “With Heidi, I can ensure that it’s started and then I can sit down, have a face to face with the patient.” Even vague language is captured accurately. “I’m so baffled by how Heidi somehow picks up what we’re even talking about when we’re pointing here or there.” When the visit ends, clinicians simply stop the transcription, select their template, and proofread. “You wait about anywhere from five to seven seconds and then the whole note is generated right there,” said Dr. Randall. “95% of the time I don’t even have to edit,” added Dr. DeRoche.

Because he no longer needs to type, he reaches the exam five minutes earlier than before. “I can get into treatment so much faster, and it streamlines the visit in a way to where there’s no breaks.” Dr. DeRoche keeps it on from the moment a patient walks in. “There’s no dead space… It really just kind of catches everything.”

He also recalls the first time he used Heidi on a patient, a moment that confirmed its potential. “The very first patient I used it on was a physician assistant at another office in a concierge practice. We both sat there and looked at the note after the visit and were blown away. She said, ‘I’ve got to go back and tell my doc what we just did.’” That first experience was a turning point. “Just seeing how accurate it was,” said Dr. DeRoche, “we were both amazed. It captured everything I needed without me touching the computer.” Both clinicians now finish their notes before leaving the clinic.

Impact

The results have been transformative both at work and at home. Across Airrosti, 6.68 million minutes of clinical conversation have been transcribed through Heidi. Together, clinicians have saved over 15,000 hours of administrative time—hours now spent on patient care, education, and family life. “It’s so much more efficient,” said Dr. DeRoche. “That’s allowing me to see two additional patients a day without an additional workload.” He estimates “hundreds of hours” saved each year.

Dr. Randall’s gains go beyond the clinic. “When I get home, I need to be a dad. I’d have an hour and a half of charting to do before I could be a dad. And I’m a dad before I’m anything else. That’s freed me up. I pull into the garage and walk into the house [asking] what do you need me to do?”

He laughed recalling his wife’s reaction: “She goes, ‘Do you not have notes to do?’ And I said, ‘No.’ She goes, ‘It works, doesn’t it?’” Both doctors agree Heidi has changed their relationship with patients too. “Patients feel like you’re more present in the visit,” said Dr. DeRoche. “They feel like you’re right there with them as opposed to behind a computer screen.”

“It’s made me a better husband. It’s made me a better father. It’s made me a better practitioner” said Dr. Randall.

And for both doctors, there’s no going back. “I can’t ever imagine going back to not using Heidi again,” said Dr. DeRoche. “I’m totally happy with it. It’s only made me better,” said Dr. Randall.
