Patient Name: Eleanor Vance Clinician Specialty: Audiologist Eleanor Vance was seen for a hearing aid check of her ReSound ONE 9 miniRIE hearing aids. She reported that the sound quality was intermittent in her left hearing aid and that she occasionally heard a whistling sound, especially when eating. Otoscopy: Otoscopy unremarkable bilaterally. The following hearing aid maintenance was performed: hearing aid(s) were cleaned and checked; domes and wax traps were replaced on the both device(s). The sound quality of the hearing aid(s) was assessed and found to be concerning based on listening checks and patient feedback. Ran HAs in Redux which revealed 0.5 microliters of moisture being removed. Due to the patient’s concerns of intermittent sound in the left hearing aid and occasional feedback, the following adjustments, changes, and recommendations were made: - A 2dB reduction was made to the high-frequency gain in the left hearing aid to address feedback. - Counselling was provided on proper insertion techniques and how to clean the hearing aids at home. - New ear moulds were recommended for the left ear to improve fit and reduce feedback, and an impression was taken. It was recommended that Eleanor return in three months for hearing aid follow-up, and she was advised to call to make an appointment when convenient. Patient was encouraged to return sooner if any further problems or concerns arise. An after-visit summary was provided.
