This patient has a history of well-controlled hypertension for the past 5 years and a family history of ischaemic heart disease in her father. Current medications include: Amlodipine 5mg once daily, Ramipril 2.5mg once daily. Allergies: Penicillin (rash). Social History: Lives with her husband. Works as a school teacher. Non-smoker. Drinks alcohol occasionally, 2-3 units per week. No illicit drug use. Clinical Findings: * ECG: Sinus rhythm with occasional premature atrial contractions, otherwise unremarkable. * Blood Pressure: 130/85 mmHg (clinic reading). * Physical Examination: Normal heart sounds, no murmurs, clear lungs. Peripheral pulses are strong and equal. * Blood tests: Normal FBC, U&Es, LFTs, and TFTs. Lipids: Total Cholesterol 5.2 mmol/L, HDL 1.1 mmol/L, LDL 3.5 mmol/L. Current Management: * Patient advised to monitor symptoms and keep a diary of palpitations. * Encouraged to maintain a healthy lifestyle, including regular exercise and a balanced diet. * Follow-up ECG planned in 3 months if symptoms persist or worsen. Specific Questions/Concerns: * Given the recurrent nature of the palpitations and her family history, what further investigations would you recommend at this stage? * Is there a role for a Holter monitor or event recorder? * Are there any specific medication adjustments that should be considered? I would appreciate your expert opinion regarding: - The need for referral for specialist cardiology assessment versus ongoing primary care management. - Potential therapeutic interventions for symptomatic relief.
