PMH, PSH, MEDS, ALLERGIES, SH, and FH: - Information reviewed with patient and in EMR, with changes made where appropriate. - PMH: Hypertension, Type 2 Diabetes - PSH: Appendectomy - MEDS: Lisinopril, Metformin - ALLERGIES: Penicillin - SH: Non-smoker, occasional alcohol use - FH: Father with history of prostate cancer **"ASSESSMENT AND PLAN:"** The patient is a 58-year-old male presenting with symptoms suggestive of a urological condition. 1. Frequent urination (R35.0) - Assessment: Likely due to bladder irritation. - Plan: Prescribe anticholinergic medication to reduce symptoms. - Counseling: Discussed the potential causes and management of frequent urination. 2. Hematuria (R31.9) - Assessment: Possible urinary tract infection or other underlying condition. - Plan: Order urinalysis and urine culture. - Counseling: Explained the importance of identifying the cause of blood in urine. 3.
