SP Progress Note Therapy Session Subjective + Observations: Patient presented with a positive attitude and readily transitioned to the therapy room. The patient demonstrated 80% accuracy in producing the /s/ sound in initial word positions. The patient was able to follow two-step directions with minimal prompting. The patient was able to construct sentences of up to four words with some support. Recommendations to the parent included continuing to practice the target sounds at home using the provided flashcards and incorporating the two-step directions into daily routines. Observed behaviours included the patient's ability to maintain focus for the duration of the session and a positive response to verbal praise. Plan Follow up session scheduled for 8 November 2024.
