Sarah Miller 123 Health Lane Wellness City, WC1 2AB Mr. David Chen 456 Serenity Avenue Peaceful Town, PT7 8XY 1 November 2026 Re: Mr. David Chen BlueSky Health Plan - Gold Tier Dear Appeals Department, Please accept this letter as Mr. David Chen's appeal to Horizon Insurance Co.'s decision to deny coverage for intensive outpatient therapy (IOP) for Generalised Anxiety Disorder (GAD). It is my understanding based on your letter of denial dated 15 October 2026 that this procedure has been denied because: "The requested intensive outpatient therapy (IOP) is deemed not medically necessary as less intensive forms of therapy have not been fully exhausted." As you know, Mr. David Chen was diagnosed with Generalised Anxiety Disorder (GAD) on 1 April 2026. Currently Dr. Emily White believes that Mr. David Chen will significantly benefit from intensive outpatient therapy (IOP). Please see the enclosed letter from Dr. Emily White that discusses Mr. David Chen's medical history in more detail. Mr. David Chen believes that you did not have all the necessary information at the time of your initial review. Mr. Chen has previously engaged in weekly individual psychotherapy for six months with minimal sustained improvement in his GAD symptoms, as documented by his GAD-7 scores, which remain consistently elevated. He has also attempted two different antidepressant medications under the supervision of his psychiatrist, Dr. Alex Green, neither of which provided adequate symptom control. Dr. Green's consultation report, dated 20 October 2026, strongly supports the medical necessity of IOP due to the severity and chronicity of Mr. Chen's symptoms and the failure of less intensive interventions. Also included are detailed session notes from his previous therapist, Dr. Laura Brown, showing the lack of progress with weekly outpatient sessions, and a recent psychiatric evaluation report confirming the severity of his GAD. Based on this information, Mr. David Chen is asking that you reconsider your previous decision and allow coverage for the procedure Dr. Emily White outlines in his letter. The delay in receiving appropriate care for his GAD is significantly impacting Mr. Chen's ability to maintain employment and daily functioning; therefore, timely approval is critical. Should you require additional information, please do not hesitate to contact Mr. David Chen at 07700 900345. Mr. David Chen will look forward to hearing from you in the near future. Sincerely, Sarah Miller
