History: John Doe has been experiencing lower back pain for the past six months, which has progressively worsened. He reports difficulty in performing daily activities and has a history of a herniated disc diagnosed two years ago. He has undergone physical therapy in the past with minimal relief. John is currently taking ibuprofen as needed and has no known allergies. He works as a software engineer and leads a sedentary lifestyle. Examination Findings: On examination, there is tenderness in the lumbar region with restricted range of motion. Muscle spasms were noted in the lower back. Investigations: An MRI of the lumbar spine shows degenerative disc disease at L4-L5. Impression: The clinical impression is chronic lower back pain likely due to degenerative disc disease. Management Plan: I have advised John to continue with gentle stretching exercises and have prescribed a course of osteopathic manipulative treatment to alleviate pain and improve mobility.
