Urology Office Consult

Thank you for the referral of John. We met today regarding his persistent urinary symptoms. Reason for Referral: John was referred due to his ongoing urinary frequency and nocturia. History of Present Illness: John has been experiencing increased urinary frequency, voiding approximately 10 times during the day and 3 times at night. He reports urgency and occasional dysuria. He also notes a weak urinary stream and incomplete bladder emptying. These symptoms have been present for the past six months and have progressively worsened. Past Medical History: Hypertension, appendectomy in 2010. Medications: Lisinopril 10mg daily, Vitamin D supplement. Allergies: NKDA. Social History: John is a non-smoker, consumes alcohol socially, and works as an accountant. Family History: Father had prostate cancer diagnosed at age 65. Physical Examination: Abdominal examination revealed a distended bladder. Digital rectal examination showed an enlarged prostate. Investigations: PSA level was elevated at 6.5 ng/mL. Ultrasound of the bladder showed post-void residual volume of 150 mL. Assessment: John's symptoms and examination findings are suggestive of benign prostatic hyperplasia (BPH). We discussed the diagnosis and potential treatment options, including medication and surgical intervention. The risks and benefits of each option were reviewed. Plan: 1. Start Tamsulosin 0.4mg daily. 2. Schedule follow-up in 4 weeks to assess symptom improvement. 3. Consider urodynamic studies if symptoms persist. 4. Referral to a dietitian for dietary modifications.
