Problems - * Recurrent chest infections * Failure to thrive * Suspected developmental delay Weight 10.5 kg Height 80 cm Parents' Concerns/History - Mother reports concerns about the child's slow weight gain and frequent chest infections. Father expresses worries about the child's speech development and social interaction. Birth history and past health history - Born at term via spontaneous vaginal delivery. No complications during pregnancy or delivery. Previous hospitalisation for bronchiolitis at 6 months old. Immunisations up to date. Family history - Father has a history of asthma. Mother has no significant medical history. No family history of developmental delay. On examination in the clinic - * Weight below the 3rd centile. * Height on the 10th centile. * Mild chest recession noted. * Poor eye contact. * Limited vocabulary for age. Developmental history - * Delayed speech development. * Limited social interaction with peers. * Gross motor skills appear age-appropriate. Plan and follow up - * Referral to a paediatric respiratory specialist for further investigation of recurrent chest infections. * Referral to a speech and language therapist for assessment and intervention. * Dietary advice and support for parents regarding feeding and weight gain. * Review in 6 weeks.
