15 Month Well Baby (AWJ)

Emma is here for a 15 month old well child check. No acute problems or concerns today. Review of Diet/Nutrition: Emma has a good appetite, has good variety in diet. Eating table foods with adequate servings of protein containing foods, iron rich foods, fruits, and vegetables. Does not consume sugary drinks. Milk intake is appropriate. Takes a multivitamin. Review of Elimination: No issues, no constipation. Review of Sleep: No issues. Has a regular bedtime. Sleeps through the night in own bed. Dental Hx: Has been seen by a dentist in the last 6-12 months. Had fluoride varnish at that visit. Brushes teeth x2/day. Takes a daily fluoride supplement. Development: Parent has no concerns about child's development or behavior. Emma is meeting the following milestones: - Imitates scribbling, drinks from a cup - Points to ask for something or to get help - Looks around when you say things like “where’s your ball?” and “Where’s your blanket?” - Uses 3 words other than names, speaks in sounds, follows spoken directions - Squats to pick up objects, crawls up a few steps, runs, makes marks with a crayon Exam: Normal growth parameters, alert and active, no abnormalities noted. Assessment and Plan: 1. Continue current diet and nutrition plan, ensure balanced meals. 2. Maintain regular sleep schedule. 3. Continue dental hygiene practices, follow up with dentist as recommended. 4. Encourage developmental activities such as reading and interactive play. 5. Schedule next well child visit in 3 months.
