Client Information Client Name: John Doe Date of Session: 1 November 2024 Provider Name and Credentials: Dr. Emily Carter, PhD Session Start Time: 10:00 AM Session End Time: 11:00 AM Session Duration: 1 hour Target Behaviors - Aggressive outbursts: Defined as hitting or kicking others. - Self-injurious behavior: Defined as head-banging or biting self. Baseline Data The frequency of aggressive outbursts was recorded at an average of 5 times per session, while self-injurious behavior occurred approximately 3 times per session. Intervention Strategies During the session, a combination of positive reinforcement and antecedent modification was employed. The behavior plan included providing verbal praise and tangible rewards for non-aggressive behavior, as well as redirecting the client to a calming activity when signs of agitation were observed. Antecedents/Triggers Aggressive outbursts were often preceded by loud noises or unexpected changes in routine. Self-injurious behavior was triggered by frustration during task completion. Response to Intervention There was a noticeable reduction in aggressive outbursts, with only 2 occurrences during the session. Self-injurious behavior was reduced to 1 occurrence, and the client demonstrated improved latency in responding to calming techniques. Recommendations It is recommended to continue using positive reinforcement and to further explore environmental modifications, such as noise-cancelling headphones, to mitigate triggers. Regular follow-up sessions are advised to monitor progress and adjust strategies as needed.
