FRACTURE CLINIC
Working Diagnosis: Left distal radius fracture, 28 October 2024, fall onto outstretched hand (FOOSH) mechanism.
PLAN:
1. Continue non-weight bearing on left wrist.
2. Maintain current cast for another 3 weeks.
3. Commence gentle finger and elbow range of motion exercises.
4. Arrange follow-up appointment for cast removal and review with orthopaedics.
5. Prescribe paracetamol 1g QDS PRN for pain.
SUMMARY:
Today: Patient presented today for their initial follow-up after sustaining a left distal radius fracture. The patient reports pain is well-controlled with paracetamol and denies any neurovascular compromise or increased swelling. Cast remains intact and comfortable.
Examination: Left upper limb: Cast intact, no signs of skin irritation or pressure areas. Capillary refill time <2 seconds in all digits. Sensation intact to light touch in radial, median, and ulnar nerve distributions. No tenderness over uncasted areas. Full, pain-free range of motion in shoulder and elbow. Fingers show good active and passive range of motion.
