Recognised Disability, Other Conditions & Current Health:

There is no history of mental health issues or substance use. The patient has not been hospitalized recently.
Medication Management:
The patient is on Baclofen for muscle spasticity, which is administered orally. Medications are stored in a secure cabinet and prescriptions are managed by the primary care physician.
Social History and Supports:

Personal Activities of Daily Living:
The patient requires assistance with showering and dressing, using adaptive clothing. They can feed themselves with modified utensils and have a consistent sleep pattern.
Instrumental Activities of Daily Living:
The patient participates in meal planning with assistance and can prepare simple meals. They require help with grocery shopping and laundry tasks.
Home Environment:
The patient lives in a single-story home owned by their parents, with three bedrooms and two bathrooms. The living area is accessible with a ramp.
Goals:
The patient's goals include improving independence in daily living activities and enhancing mobility. Occupational therapy will focus on fine motor skills and adaptive techniques, with sessions twice a week for six months.
