MH: - Taking ibuprofen as needed. - Allergies: Penicillin. Examination: - Visual inspection shows redness and swelling around the upper right molar. - Palpation reveals tenderness in the affected area. - X-ray indicates possible infection at the root tip. - Condition of teeth: multiple fillings, gums appear inflamed. - Signs of infection: pus discharge from the gum line. Rads Taken: X-ray shows periapical radiolucency indicating infection. Diagnosis - Acute periapical abscess secondary to failed root canal treatment. - Contributing factors: poor oral hygiene and smoking. TX options discussed: Antibiotic therapy, re-treatment of the root canal, or extraction of the affected tooth. Plan: - Prescribed amoxicillin 500mg three times a day for 7 days. - Scheduled re-treatment of the root canal in 1 week. - Advised to quit smoking and reduce sugar intake. - Follow-up appointment in 2 weeks to assess healing and treatment efficacy. - Educated patient on maintaining oral hygiene and regular dental check-ups to prevent future issues.
