Thank you for seeing John Doe for further evaluation and management. He presented to Emergency Department today (01 November 2024) with sudden onset chest pain radiating to his left arm, associated with shortness of breath. He reported the pain started approximately 30 minutes prior to arrival.
Mr. Doe's relevant medical history includes hypertension and a previous myocardial infarction in 2018 and is currently using Aspirin 75mg daily, Lisinopril 10mg daily, and Atorvastatin 20mg daily.
Assessment: Suspected Acute Myocardial Infarction.
Relevant examination findings included an elevated heart rate of 110 bpm, blood pressure 160/90 mmHg, and ST-segment elevation in leads II, III, and aVF on ECG.
Investigations and management done in the emergency department included an ECG which showed ST-segment elevation in leads II, III, and aVF. Cardiac enzymes were drawn, showing elevated Troponin levels. He was given Aspirin 325mg, and IV morphine for pain control. He was also started on oxygen and placed on cardiac monitoring.
